Skyline's Eye Clinic offers free basic eye exams performed by volunteer eye doctors. In addition, Skyline's Optician Clinic is a chance for patrons with a valid prescription to choose affordable or even no-cost eyeglass frames.
To schedule an Eye Exam:
There are two ways to schedule an Eye Exam:
1. Fill out the above form OR
2. Call Skyline at (405) 601-6657 and leave a message requesting a call back. If you do not leave a voicemail, the Eye Clinic cannot set up an appointment.
If you are 18 years old or older, you need to make your own appointment. Parents can only schedule appointments for children under 18.
Important Information:
Keep in mind that there is a high demand for appointments and limited space in the building. There is a waitlist for Eye Clinic appointments.
Check the calendar to find out when the eye clinic will be held.
Write down the date, time, and location of the clinic where the eye exam will take place.
We try to make reminder calls a few days before the eye exam, but it is not guaranteed. If you do not show up for your appointment, you will not be able to reschedule for two years.
Make sure to attend the eye exam on the scheduled date.
If you need to cancel your appointment, you must give a 24-hour notice in order to reschedule.
If you do not, you will have to wait for two years before you can schedule another appointment.
Preparing for your appointment:
Take notes on the date, time and location of the clinic that will be performing the eye exam.
While reminder calls are often issued a few days prior to the eye exam, no show appointments will not be able to reschedule for two years.
If your appointment is at Skyline:
Arrive at the pedestrian gate at 9 AM on the day of your appointment.
TO PURCHASE GLASSES:
Obtain a recent prescription that is less than two years old. This can be from either a Skyline appointment or an outside clinic.
Ordering through the Optician Clinic is only available on Wednesdays from 9:00-11:30 AM.
The Eye Clinic Manager will visit with the patron to help select and fit the frames.
Glasses cost $45 for single lenses and bifocals.
Those that cannot afford this cost may speak with the Eye Clinic Manager to see about receiving assistance.
All payments must be made in full before frames are sent to the lab to be made.
Return the following Wednesday (unless otherwise notified) to pick up frames and receive any adjustments that may be necessary.
Please note, eye doctors will not be present on Wednesdays. EYE EXAMS WILL NOT BE DONE DURING OPTICIAN CLINICS.
